Employers are to file this form for reporting the amount of income taxes, Social Security taxes, and Medicare taxes withheld from an employee’s wages. If the employer makes a payment with this form, the attached voucher needs to be completed as well. Form 941 must be filed once every three months. While completing the form, the employer should add information about their business entity and indicate the filing period. The Employer’s Quarterly Federal Tax Return must be dated and signed by the person authorized to do so.
